Dog Boarding | Daycare | Training Application Form

For the safety of our staff and canine guests; we require all new-comers to complete our application form that allow camper parents the opportunity to pre-determine if Camp Miracle Paws will be a good match for their dog.

Once we receive your dogs application, we will schedule a free meet and greet tour for parents and their dogs to come and sniff around our camp, ask questions and we will accommodate a training session if you are intersted in our training programs.

To help dogs to better acclimate for stays longer than a day; we require all new comers to sign up for a intro daycare session.  This can be scheduled on the same date as the meet and greet, or we can schedule another day for drop off.

Depending on your dog’s comfort level, we might ask that your dog attend one or more short term intro daycare visits while gradually building up on the duration before booking a longer stay. Once your dog understands that Camp Miracle Paws is a fun place to be, he will have a much better time relaxing while mingling with a few other dogs that have gone through the same application process.

Camp Miracle Paws is not a good fit for dogs that are:

  • aggressive with humans
  • aggressive with other dogs: depending on several variables, we might be able to accommodate some anti-social dogs that are under 35lbs, please call to discuss your dog’s personality and special needs.
  • escape artists- please know that our fencing is 4-5′ tall. Camp Miracle Paws is not a good fit i f your dog is known to escape over or under fencing
  • anxious- if your dog suffers from separation anxiety and cannot settle into a confined space, we can offer training with the owner to help the dog learn to relax
  • pregnant, in heat, has known contagious conditions including skin rashes, diarrhea, ticks and fleas
  • you must have owned your dog for at least two weeeks prior to completing this form allowing you time to get to know your dog’s personality and to help assure that the dog is not contagious

The information on this form only applies to the dogs that will be utilizing our services.

  • Depending on several variables, we might be able to accommodate small to medium sized anti-social dogs. Please call to discuss your observations prior to completing the form.
  • From 7am-9pm; Every two hours, our canine guests are allowed out of their spacious private cabins so they can play, mingle, relax and explore.
  • If you are interested in boarding or daycare, please plan to leave your dog for a few daycare intro hours allowing him time acclimate to our facility.
  • Please provide us with your ETA's for each as well- Example: Drop off: Jan 4 @ 2pm PU- Jan 7@10am
  • Laid back, energetic, happy, sad, anxious, nervous, fearful, excitable, serious, shy, playful, goofy, instigator, happy go lucky, nosey, mischievous, destructive, ect-
  • We cannot accept pregnant or in heat females.
  • Please provide some brief history on the dog- and if your dog is destructive or has eaten objects before, please let us know.
  • If you only have one dog, please skip down to the next required field.
  • Laid back, energetic, happy, sad, anxious, nervous, fearful, excitable, serious, shy, playful, goofy, instigator, happy go lucky, nosey, mischievous, destructive, protective, ect-
  • No pregnant or in heat females are allowed. And no in tact males over the age of one.
  • Does your dog like to eat things other than food or treats? Please let us know at each drop off as your dog will be exposed to things typically found in a country home interior and exterior setting.
  • Current copies are required for your dog to visit or to stay with us. You can upload a copy below, or you or your vet can email a copy to: info@miraclepaws.org. Dog Vaccinations: DPP* 1 or 3-yr. vaccine required *Distemper, Parvovirus & Bordetella, Rabies- 1 or 3-yr. vaccine required All dogs must be on monthly Heartworm and Fleas & Tick preventatives.
  • In the event that your dog escapes or is found to be or becomes contagious, sick or injured while in our care you and your agent will be called as soon as possible. Depending on the situation, we have the right to take your dog to a local emergency veterinarian of our choice and you agree to be responsible for all expenses for boarding and care that they or a specialist veteranarian provides to your dog. You also agree to be responsible for all expenses for any other dogs that your dog caused injury or illness to.
